Loyola Chiropractic Payment Policy
Our goal in this office is to provide you, the patient, with excellent chiropractic care. We want you to be comfortable dealing with financial matters, and believe open communication will enhance the positive outcome we all desire.
All fees are due on the day services are rendered. For your convenience we accept Visa, Master Card, and American Express credit cards.
Changes to appointments must be made by telephone at least 24 hours prior to the appointment time on the prior business day to avoid the regular fee. Missed appointments will be charged in full.
If you have chiropractic insurance, we are interested in you receiving the maximum benefits. As an added service, we will prepare your insurance claim for you and forward it to your insurance company. All insurance benefits will be paid directly to you by your insurance company.
Our doctors are not members of any HMO, PPO, Medicare or any other provider networks. Therefore, any coverage you may have for services provided in this office will be deemed “out of network coverage” by your insurance company.
